Key Responsibilities

  1. Coordinate the complete transplant process for liver transplant patients from initial evaluation through transplantation and follow-up.
  2. Act as the primary point of contact between patients, families, transplant surgeons, hepatologists, physicians, nursing teams, and other allied departments.
  3. Coordinate pre-transplant evaluation, including consultations, diagnostic investigations, laboratory tests, imaging, and specialist assessments.
  4. Maintain and update patient records, transplant evaluation documents, investigation reports, and other relevant medical documentation.
  5. Coordinate donor evaluation and ensure that all required investigations, consultations, and documentation are completed within the defined timelines.
  6. Facilitate communication and coordination between the transplant team and patients/families regarding appointments, investigations, treatment plans, and transplant-related procedures.
  7. Coordinate transplant-related admissions, OT scheduling, bed availability, investigations, and other operational requirements.
  8. Ensure that all necessary statutory, regulatory, consent, and transplant-related documentation is completed and maintained as per hospital and regulatory requirements.
  9. Coordinate with departments such as Radiology, Laboratory, Blood Bank, Pharmacy, ICU, OT, Anaesthesia, Finance, and Medical Records for seamless patient care.
  10. Assist the transplant team in maintaining transplant waiting lists, patient status updates, and relevant transplant databases.
  11. Track patients during the pre-transplant and post-transplant phases and coordinate scheduled follow-up consultations, investigations, and reviews.
  12. Educate patients and their families regarding the transplant process, investigations, appointments, medication compliance, and follow-up requirements as advised by the clinical team.
  13. Maintain regular communication with patients and families to provide updates regarding appointments, reports, transplant readiness, and other administrative requirements.
  14. Monitor pending investigations, consultations, reports, and documentation and follow up with concerned departments to ensure timely completion.
  15. Support the transplant team in maintaining accurate statistics, MIS reports, transplant activity reports, and departmental records.
  16. Coordinate with the finance and insurance teams regarding approvals, estimates, documentation, and other transplant-related administrative requirements.
  17. Assist in organizing transplant-related meetings, multidisciplinary discussions, patient reviews, and case presentations.
  18. Ensure confidentiality and secure handling of patient medical records and transplant-related information.
  19. Identify and escalate delays, patient concerns, clinical or administrative issues to the concerned transplant team members promptly.
  20. Perform any other transplant coordination and patient-support responsibilities assigned by the Transplant Unit Head or hospital management.
