Responsibilities
- Conduct comprehensive mental health assessments to determine client needs and eligibility for services.
- Develop and implement individualized treatment plans in collaboration with clients and interdisciplinary team members.
- Provide crisis intervention and support to clients in community settings to ensure safety and stability.
- Collaborate with psychiatrists, nurses, social workers, and other team members to coordinate care.
- Monitor client progress and adjust treatment plans as necessary to achieve desired outcomes.
- Facilitate client access to community resources such as housing, employment, and healthcare services.
- Maintain detailed documentation and comply with all regulatory and agency standards.
- Hold a valid QMHP certification or equivalent licensure as required by state regulations.
- Minimum of a bachelor’s degree in psychology, social work, counseling, or a related field.
- More than 1 year of experience working with individuals diagnosed with serious mental illnesses within community-based programs is a plus.
- Strong knowledge of ACT model principles and evidence-based mental health practices.
- Experience engaging clients and interdisciplinary team members in a mental health care setting.
- Ability to work independently and manage multiple mental health cases with flexibility and professionalism.
- Must be eligible to work in the United States.
- Willingness to provide services in community settings, including client homes and other locations as needed.
- Annual salary: 55000-62000 USD/year
