About the Role

We are seeking an experienced Care Manager, responsible for the day-to-day operational management of a defined portfolio of complex healthcare packages. Reporting to the Operations Manager, the postholder has delegated responsibility for ensuring safe, effective, person-centred and operationally efficient service delivery.

 

The role acts as the primary operational link between clients, families, commissioners, clinical teams, recruitment and the wider organisation, while providing day-to-day leadership to the care teams supporting allocated services.

 

Key Responsibilities

  • Manage complex healthcare packages, ensuring safe, compliant and person-centred care.
  • Oversee package mobilisation, workforce planning, rotas and service changes, ensuring continuity of care.
  • Monitor service performance, resources and operational risks, taking corrective action where required.
  • Manage incidents, safeguarding concerns and service disruption, including Duty of Candour and appropriate escalation.
  • Apply the Mental Capacity Act, including Best Interests, DoLS/LPS and least restrictive practice.
  • Work collaboratively with clients, families, clinical teams, commissioners and other stakeholders.
  • Maintain accurate records and support CQC compliance, governance, audit and continuous improvement.

 

People Management Responsibilities

  • Provide day-to-day leadership, coaching and support to colleagues, promoting accountability and high standards.
  • Manage performance, attendance, conduct and development fairly and consistently.
  • Set objectives, support succession planning and identify development opportunities.
  • Follow HR policies and work with HR on people-related matters.
  • Promote company values, safeguarding, inclusion and collaborative working.

 

Outcomes & Performance Measures

  • Safe, compliant and person-centred delivery maintained across the allocated portfolio.
  • Commissioned hours, rota coverage and continuity standards achieved, with workforce risks escalated promptly.
  • New packages mobilised with confirmed workforce, operational and service readiness.
  • Operational incidents, complaints and safeguarding concerns recorded, managed and escalated within required timescales.
  • Package KPIs, service standards and agreed corrective actions delivered.
  • Accurate and timely client, workforce and operational records maintained in relevant systems.
  • Avoidable agency use, overtime and operational costs actively controlled through effective workforce deployment.
  • People-management standards met, including objective setting, attendance management, performance management and accurate HR-system records.

 

Background and Qualifications

Required:

  • Experience coordinating or managing regulated health or social care services, ideally complex healthcare packages delivered in the community.
  • Experience of rota management, workforce planning and maintaining continuity of care.
  • Experience of leading and supporting colleagues, including managing attendance, conduct and performance.
  • Working knowledge of safeguarding, incident management, the Mental Capacity Act 2005 and person-centred care.
  • Strong communication, stakeholder-management, organisation and problem-solving skills.
  • Ability to maintain accurate records, use care-management and HR systems, manage competing priorities and escalate risk appropriately.

 

Desired:

  • Relevant health, social care or management qualification.
  • Knowledge of CQC requirements, Duty of Candour and community complex-care governance.
  • Experience of package mobilisation, audit activity and continuous service improvement.
