Weekend Clinical Intake Coordinator (LVN / RN)  Full-Time Role; Saturday - Sunday & Monday - Wednesday  Office Locations: Burlingame · San Jose · Walnut Creek · Sacramento     About the Role This is a unique opportunity for a clinically experienced LVN or RN to step into a high-impact operational role at the agency level. As our Weekend Clinical Intake Coordinator, you will serve as the central point of contact for our entire organization on Saturdays and Sundays, managing Kaiser referral intake, supporting field clinicians with clinical escalations, and ensuring our patients receive timely, seamless care coordination when our weekday leadership team is offline. This role is ideal for a clinician with home health experience who thrives in a fast-paced, autonomous environment, communicates confidently with both clinical and operational partners, and can triage competing priorities with clarity and calm.
