- Baccalauréat en travail social
- Permis de conduire valide
- Formation OCCI
- Formation en évaluation psychosociale dans un contexte de régime de protection
- CV à jour
- Voiture requise
Principales responsabilités
- Évaluer les besoins des usagers à domicile et mettre en place les services nécessaires au maintien à domicile
- Offrir un soutien psychosocial aux usagers
- Accompagner et soutenir les proches aidants
- Collaborer avec les différents intervenants afin d'assurer la continuité des services
