Rôle

Le ou la travailleur(euse) social(e) intervient auprès d'une clientèle vivant à domicile afin d'évaluer leurs besoins, favoriser leur maintien à domicile et offrir un soutien psychosocial aux usagers et à leurs proches.

Tâches principales

  • Évaluer les besoins des usagers à domicile.
  • Élaborer et mettre en place des plans d'intervention adaptés.
  • Offrir un soutien psychosocial aux usagers et aux proches aidants.
  • Effectuer les évaluations psychosociales dans un contexte de régime de protection.
  • Collaborer avec l'équipe interdisciplinaire et les partenaires du réseau.
  • Assurer le suivi clinique et la rédaction des dossiers.

Exigences

  • Membre en règle de son ordre professionnel.
  • Baccalauréat en travail social.
  • Minimum 2 ans d'expérience comme travailleur(euse) social(e).
  • Formation OCCI.
  • Formation pour réaliser des évaluations psychosociales dans un contexte de régime de protection.
  • Permis de conduire valide.
  • Véhicule requis.
