- Utilize effective communication skills in reporting pertinent information on time.
- Adhere to telephone etiquettes.
- Maintain professional decorum and communicate in English/local language in the areas of work with colleagues.
- Adhere to the grooming standards laid down by the organization.
- Evaluate the effectiveness of nursing care on patient condition.
- Inform senior nursing professional about current activities of the unit and report unusual occurrence, incidents or other problems and also follow up as needed.
- Utilize the man power and resource provided effectively towards patient care,
- Report accurately by effective communication about patient condition and intervention.
- Ensure all documentation done are appropriate with clinical terminology on time.
- Perform independent problem solving and decision making in a logical and deliberate manner and escalate it to senior nursing professional.
- Exhibit flexibility and adaptability in meeting the needs of the patients and the workplace.

NURSING ADMISSION ASSESSMENT
- Perform comprehensive nursing assessment on admission patients within 30 Minutes with consideration to clinical condition, pain, activities of daily living, allergies, integrity, valuables, safety needs, age, mental, social and physical status etc.
- Identify patient problems based upon data collected through nursing assessment as well as information obtained through the family. Take measures to address any vital concerns compromising the haemodynamic stability and safety issues.
- Follow all aspects of the hospital policy in terms of safety, personal belongings, medico – legal entity and medications.
- Assess vital signs, important information and report abnormalities to the doctor and carry out orders as per instructions.
- Document the nursing assessment and patient problems using appropriate clinical terminology.
- Assess and communicate to the concerned about educational, spiritual, pain management, dietary and special needs.
- Involve the family members in decision making andfuture plan of patient care by effective communication and education.
PLAN OF CARE
- Formulate nursing care plan in context to patient needs, safety, PFE, diet and other needs in each shift considering the age, behavior and concerns identified through nursing assessment.
- Ensure that care is provided with a comprehensive approach always ensuring safety and nursing clinical outcome.
- Establish priority for nursing needs based on nursing assessment, problems identified and estimated length of stay.
- Active participation in the development of interdisciplinary plan of care as per policy.
- Ensure timely and continuity of care with adequate communication.
- Ensure accurate documentation.
PROVISION OF NURSING CARE
- Practice within the general policies, philosophy and objectives of the department of nursing and the hospital with focus on safety, quality and infection control
- Provide nursing care in a therapeutically efficient manner by adapting current practices demonstrating social, clinical and professional judgment and competency.
- Participate in the implementation of the treatment plan utilizing intervention appropriate to the needs of the patient.
- Demonstrate knowledge and skills related to treatment, procedures, precautions, seclusions and restraints .
- Provide supportive counseling of patient and family for current patient concerns.
- Assess unit milieu and maintains a safe, therapeutic environment for patients, family members and staff.
- Assess and meet daily living needs of patients in dressing, bathing, grooming, toileting, eating and sleeping.
- Identify all vulnerable patients and ensure that the safety measures are followed at all time.
- Demonstrate ability to recognize different types of incidents and reporting mechanism for such issues.
- Document nursing interventions timely and accurately.
- Provide ongoing patient / family education according to the individual needs of the patient and record same in the patient file.
- Use and maintain biomedical equipment safely.
- Update and keep abreast of the HAI statistics control measures and practices those in the working environment.
- Participate in discharge planning, education and care at home along with patient and family actively.
- Provide culturally competent care to the patient
MEDICATION MANAGEMENT
- Knowledgeable about the medication process, (Adverse Drug Event )ADE and Adverse Drug Reaction ) ADR
- Accurate transcription of medication and treatment orders for the indenting process.
- Recognize improper or inadequate orders and reports to supervisors and physician before proceeding
- Check and receive medication against the order during pharmacy delivery.
- Appropriate and safe administration of prescribed medication and treatment using medication policy of 6 Right’s of medication, 2 identifiers, allergic history, (double check and double acknowledgement for applicable medicines), admixture,inform Clinical Pharmacist/Duty doctor and follow outside medication and self-medication policy etc.
- Maintain accurate medication administration record.
- Monitor the patient post administration for any adverse events if there is a event report and complete the event form With in 24 hrs
- Demonstrate safe storage and control of medication.
- Evaluate and document patient response to medication regimen.
- Implement the waste management policy
- To be knowledgeable of action, side effects proper dosage and contraindication of drugs being administered.
- Need to demonstrate the knowledge on documentation requirement for proper administration of medication those protesting patients.
- Identify the need for administration of SOS medication and document it accurately.
- Teaching on medication management to patient and family at home as per advise by the consultant.
- Demonstrate knowledge & competency and comply with the protocol of high alert and high risk medication usage (Transcribing, receiving, storing, administering, monitoring, reporting ADR and documenting).
PROFESSIONAL SAFETY, GROWTH AND DEVELOPMENT
- Assume responsibility for personal and professional growth and development through participation at in service education programme, workshop, staff meetings, unit meeting and on the job training
- Adhere to all safety policies and safe work practices.
(Sharps policy, falls, communicable disease etc.)
- Adhere to all hospital policies and procedures.
- Participate in unit staff meeting, monthly meeting with Nursing Leadership, IDTR round which will enhance the quality of services provided to patient.
- Recognize own limitation, seek information and advice when unable to solve conflict/ problems on an individual basis.
- Maintain required professional licenses and renewal of Registration as per requirement.

- To be aware and practice safety of high alert/high risk medications.
- Follow pre surgical process to eliminate wrong side, wrong patient and wrong procedure surgery.

QUALITY & SAFETY
- To be aware and participate in quality assurance program of the department & the organization.

COMPETENCY IN POINT OF CARE TESTING (POCT)
- To be knowledgeable about the basic operation of the POCT device.
- To beknowledgeable about the sampling procedure
- To be aware about the Quality controls, documentation and daily maintenance of the machine
- To beaware of validation from Laboratory
- Follow infection control practice while dealing with the equipment.
- To Check the Calibration of the machine.
- To adhere to waste management protocol as per hospital policy.
COMPETENCY IN BLOOD TRANSFUSION
- Need to be aware about the Blood Transfusion Process.
- Need to be knowledgeable about the Blood Monitoring Protocol.
- Ensures the written consent & written order for the Blood / Blood product transfusion.
- Checks and document the vital signs as per protocol and report abnormalities to the treating team and carry out orders as per instructions.
- Need to be aware about the validity of Blood products.
- Provide supportive counselling to the patient & family regarding Transfusion.
- Evaluate and document the patient’s response to the transfusion.
- Need to be aware about the blood transfusion reaction & reporting process
- To be competent in following infection control practices.

- To be knowledgeable and skill on feeding techniques / parenteral / enteral feeding.
- Be aware & practice infection prevention based on hospital guidelines including neutropenic care.
- Demonstrate knowledge of patient’s safety & implement the same .
- Be aware of resuscitation techniques & assists during emergent situations.
- Demonstrate knowledge skill in identifying and managing the code as per protocol.
- Be aware of medication used , its calculation , action adverse drug effect & adverse drug reactions.
- Provide education to patient and family based on individual’s need.

- Report any untoward incident / event to senior nursing professional.
