General Description: Under the direction of the Mental Wellness Coordinator, the Suboxone Treatment Coordinator is responsible for coordinating case management and programming for individuals engaged in treatment for Opioid Use Disorder within the community of Sipekne’katik. Working as part of a multidisciplinary team, the Suboxone Treatment Coordinator serves as a liaison between community agencies and the Sipekne’katik Opioid Treatment and Recovery Program, supporting treatment planning, case management, and targeted interventions that promote long-term recovery and wellness. This position works from a holistic, person-centred, and harm reduction approach while helping clients navigate services and overcome barriers to care.



Responsibilities:

Client Treatment Planning & Case Management

  • Complete intake and orientation sessions for new clients entering the program.
  • Develop individualized treatment plans that support emotional, physical, mental, and spiritual wellness.
  • Coordinate ongoing case management and monitor client progress toward treatment goals.
  • Identify and address barriers to treatment, including transportation, community supports, and access to services.
  • Maintain confidential client files and complete regular follow-up with participants.
  • Work collaboratively with Mental Health Clinicians and Circle of Care team members to coordinate client care.

Medication Monitoring & Support

  • Witness dispensing of buprenorphine (Suboxone) as required.
  • Conduct witnessed urine drug screening.
  • Complete medication counts in accordance with program requirements.
  • Maintain positive therapeutic relationships through regular client contact.

Community Outreach & Program Development

  • Organize community and family education related to Opioid Use Disorder and recovery.
  • Coordinate stakeholder meetings and community partnerships.
  • Foster collaboration with community agencies to strengthen recovery supports.
  • Assist with the
    development of programming that reflects Indigenous approaches to healing and wellness.

Administrative Responsibilities

  • Maintain accurate documentation and client records.
  • Prepare reports and support program evaluation.
  • Assist with funding proposals and program planning as required.
  • Perform other related duties as assigned.



Qualifications and Experience:

  • Bachelor's Degree in Psychology, Counselling, Social Work, Addiction Studies, or a related discipline. An equivalent combination of education and experience may be considered.
  • Minimum two (2) years of experience working in a clinical, medical, or community health setting.
  • Experience supporting individuals living with substance use disorders is preferred.
  • Experience working within Indigenous communities or First Nation organizations is considered a strong asset.

 

Knowledge, Skills, and Competencies

  • Knowledge of Opioid Use Disorder, Opioid Agonist Therapy, and harm reduction principles.
  • Strong understanding of person-centred and trauma-informed approaches to care.
  • Knowledge of community health resources and service navigation throughout Nova Scotia.
  • Excellent interpersonal, communication, and relationship-building skills.
  • Ability to maintain confidentiality and exercise sound professional judgment.
  • Strong organizational and case management skills.
  • Proficiency with Microsoft Office Suite.
  • Ability to work collaboratively within a multidisciplinary team.
  • Familiarity with the Sipekne’katik community is considered an asset.
  • Valid Class 5 driver's licence with access to a reliable vehicle.
  • Satisfactory Criminal Record Check and Vulnerable Sector Check
