Immanuel Pathways PACE® is seeking a Speech Language Pathologist to work at Pathways Omaha located at 5755 Sorensen Parkway, Omaha, NE 68152.

 

Schedule: PRN (as needed) daytime hours with a flexible schedule based on participant needs and therapy referrals. Hours may vary week to week. This is a great opportunity for a Speech Language Pathologist seeking supplemental income, flexible scheduling, or the potential for more consistent hours in the future.

 

Pay will be based upon experience, certifications, skills and education.

Job Duties & Skills Required

What You Will Do

The overall purpose of this job is to provide clinical speech therapy evaluation and treatment for participants on acute, restorative, or maintenance levels as needed in all Pathways settings. This job integrates the speech therapy treatment plan into the IDT plan of care. The responsibilities of this role also include assisting in the ordering, inventory, distribution and maintenance of durable medical equipment for participants as needed. The Speech Language Pathologist will abide by the philosophy, practices, and protocols of the PACE organization.

QUALIFICATIONS

Education-

  • Bachelor’s degree in Speech-Language Pathology or Communication Sciences and Disorders from an approved/accredited program required.
  • A master’s degree in Speech-Language Pathology or Communication Sciences and Disorders from an approved/accredited program preferred.
  • Completed Clinical Fellowship (CF).
  • Passing score on the Praxis Examination in Speech-Language Pathology licensure exam. 

Experience-

  • Two (2) years of appropriate experience as a speech therapist preferred.
  • One (1) year of experience working with the frail or elderly population preferred
  • Completion of job specific training related to working with the elderly population must be completed within the first six months of hire.

Other Requirements-

  • Must have a valid driver’s license, proof of insurance and have means of transportation.
  • Basic Life Support (BLS) Skills within 6 months of hire.
  • Be legally authorized (currently licensed or if applicable, certified or registered) to practice the job’s functions and actions in the respective state in which he/she is employed.
  • Only act within the scope of his/her authority to practice in the respective state in which he/she is employed.

 

KEY RESPONSIBILITIES

Key Responsibilities and Duties of the Job

Clinical Assessment & Care Planning

  • Conducts comprehensive in-person speech-language pathology evaluations for Pathways participants upon receipt of referral and collaborates with the Interdisciplinary Team (IDT) to develop individualized plans of care.
  • Performs in-person reassessments for participants on an active caseload at least every six months, or more frequently as warranted by participant condition or care needs.
  • Evaluates participant functional status and therapeutic needs, monitors treatment progress, explains recommended precautions, and discusses conditions and treatment plans with participants and the IDT.
  • Provides speech-language pathology services in accordance with physician orders and established plans of care, clarifying treatment instructions as needed and collaborating with the IDT regarding participant care concerns.
  • Applies evidence-based speech-language pathology principles and practices to prevent functional decline; address speech, cognitive, sensory, and swallowing impairments; and promote optimal participant functioning.
  • Provides evaluation and treatment services at acute, restorative, and maintenance levels of care based on participant needs.

Documentation & Clinical Communication

  • Maintains accurate, timely, and complete clinical documentation, including evaluations, progress notes, reassessments, and treatment records in accordance with organizational and regulatory requirements.
  • Monitors participant progress, evaluates outcomes, and communicates changes in condition to the IDT to support coordinated, twenty-four-hour care delivery.
  • Assists with data collection, reporting activities, and maintenance of participant statistics and other departmental records.

Quality, Compliance, & Program Support

  • Contributes to the development and enhancement of speech therapy policies, procedures, and service delivery practices.
  • Supports quality assurance activities and maintains compliance with safety, environmental, infection prevention, and regulatory standards.
  • Practices within the scope of professional licensure, certification, and applicable regulations.
  • Complies with all organizational policies and procedures, Occupational Safety and Health Administration (OSHA) requirements, Home Health Licensure regulations, and other applicable federal, state, and local requirements.
  • Protects participant, family, employee, and organizational information by maintaining confidentiality and privacy in accordance with applicable laws and policies.
  • Implements and adheres to Exposure Control and Emergency Preparedness Plans.
  • Participates in and supports quality improvement initiatives designed to enhance participant outcomes and operational effectiveness.
  • Maintains treatment areas in a clean, safe, and organized manner and ensures therapy equipment is properly maintained and operated in accordance with manufacturer recommendations.

Leadership & Professional Development

  • Participates in required staff meetings, in-service training, continuing education programs, and other professional development activities.
  • Maintains required professional licenses, certifications, and relevant professional affiliations.
  • Performs competency assessments for direct care staff as assigned or required.
