Job Summary and Responsibilities

As our RN Care Coordinator, you will oversee the progression of care and discharge planning for identified patients, promoting quality and cost-effective outcomes while adhering to all hospital policies and regulations. Every day you will coordinate, communicate, and collaborate with utilization management, nursing, physicians, ancillary departments, insurers, and post-acute providers to progress care toward optimal outcomes at the appropriate level. You will also advocate for patients and families, identifying and addressing barriers to care transitions, and striving to enhance the patient experience. To be successful in this role, you will be a Registered Nurse with strong clinical judgment, exceptional communication and collaboration skills, a deep understanding of care coordination principles, and a fervent commitment to patient advocacy and enhancing the overall patient experience.

 

  • Facilitates the development of a multidisciplinary discharge plan, engaging other relevant health team members, the patient and/or patient representative, and postacute care providers in accordance with the patient's clinical or psychosocial needs, choices, and available resources.
  • Collaborates with the healthcare team and postacute service providers to ensure timely and smooth transitions to the most appropriate type and setting of postacute services based upon the patient's clinical needs.
  • Completes and documents a discharge planning assessment on those patients identified by the designated screening process, or upon request, and reassesses the patient as appropriate to update the plan accordingly.
  • Identifies risk for readmission and implements interventions to mitigate those risks for at least a 30day period.
  • Oversees and evaluates the implementation of the discharge plan.
  • Collaborates with the multidisciplinary team to ensure progression of care and appropriate utilization of inpatient resources using established evidencebased guidelines/criteria.

Schedule: Weekend coverage and Holidays. 

Job Requirements

Required

  • Graduate of an accredited school of nursing
  • Minimum two (2) years of acute hospital clinical experience or a Master'ss degree in Case Management or Nursing field in lieu of 1 year experience
  • RN license in the state(s) covered: Arizona RN:AZ or Compact Arkansas RN:AR or Compact California RN:CA Colorado RN:CO or Compact Georgia RN:GA or Compact Iowa RN: IA or Compact Kansas RN:KS or Compact Kentucky RN:KY or Compact Minnesota RN:MN Nebraska RN:NE or Compact Nevada RN:NV North Dakota RN:ND or Compact Ohio RN:OH or Compact Oregon RN:OR Tennessee RN:TN or Compact Texas RN:TX or Compact Utah RN:UT or Compact Washington RN:WA or Partial
  • BLS required within 3 months of hiring

 

Preferred

  • Bachelor's Degree in Nursing (BSN) or related healthcare field
  • At least five (5) years of nursing experience
  • Certified Case Manager (CCM), Accredited Case Manager (ACMRN), or UM Certification
