SUMMARY
The Certified Diabetes Educator is responsible for the development, implementation, and evaluation of Diabetes programs, and staff education in the care of Diabetes clients. This responsibility includes participation in the screening of employment applicants, orientation, education, and evaluation of specialty nursing staff, and ensuring that quality patient care is delivered efficiently. Also provides clinical nursing leadership in area of specialty to Home Care professional staff.

RESPONSIBILITIES

  • Consultative - Provides consultation to the patients/PCG staff who is participating in the delivery of care for management to the high-risk Diabetes clients, and to the generalist whom is caring for the less complicated Diabetes Patient.
  • Operations - Participates in the development and maintenance of current operational policies and procedures related to Diabetes programs.
  • Training - Responsible for development and implementation of orientation, education, and training activities for agency regarding Diabetes management of persons/PCG
  • Quality Management - Collaborates with the Quality Management staff to design, monitor, and evaluate the continuous quality improvement process, and risk management components of the clinical program. Conferences, joint visits with, and convenes specialty team on a monthly basis to evaluate and update program.

REQUIRED QUALIFICATIONS

  • Completion of an accredited program of professional nursing studies
  • A valid NY RN license is required.  
  • Diabetes Educator Certification required, or ability to obtain Diabetes Educator Certification within 6 months of start date
  • Must demonstrate ongoing participation in educational opportunities prepared for National Certification Board Diabetes Educators exam every five years.  
  • Ability to travel and valid NYS driver’s license required.   
  • Must demonstrate solid interpersonal, organizational and time management skills.  
  • Basic computer skills are necessary.

Light - Clinical - Light clinical roles involve frequent standing and walking, with occasional lifting of light objects (≤20 lbs.) and routine reaching, grasping, and fine manipulation for tasks such as patient care, laboratory work, or specimen processing. Staff rely on strong visual and auditory acuity to accurately observe specimens, read instrumentation, monitor patients, respond to alarms, and communicate effectively with care teams. Cognitive demands include multitasking, prioritizing clinical and laboratory workflows, and coordinating with interdisciplinary teams. Emotional resilience is necessary due to frequent interruptions, shifting priorities, and the need for precise and compassionate interaction with patients and colleagues. Environmental exposures may include infectious materials, biological specimens, cleaning agents, sharps, chemical reagents, and wet surfaces. PPE use, TB testing, and schedule flexibility are commonly required.

The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
