Short Description

Ability to effectively triage issues by using appropriate resourcesReview discharge instructions and ensure understandingDetermine if there are questions or concerns following the hospital stay.Escalate issues or concerns by notifying appropriate parties (e.g. Case Management,    Physician, Clinical Director, Patient Relations, etc.)Summarizing responses in a database for documentation, trend analysis and follow-up.
