Full-Time | Comprehensive Benefits | Supportive Rehab Team

Looking for a PT position where you can actually feel connected to your patients and your team?

AdvantageCare Rehabilitation is hiring a Full-Time Physical Therapist (PT) to join our rehabilitation team at Pocopson Home in West Chester, Pennsylvania.

This is an opportunity to become part of an established rehab program where you'll work with an adult and geriatric population, collaborate closely with other disciplines, and have the opportunity to follow your patients' progress from evaluation through treatment and recovery.

And one of the best parts of this opportunity is the people you'll be working with.

The rehabilitation team at Pocopson is collaborative, welcoming, and genuinely invested in providing great care. You'll have an approachable Director of Rehabilitation who understands the importance of supporting clinicians while also giving them the autonomy to do what they do best.

If you're looking for a place where you can settle in, become part of the team, and make an impact, Pocopson is worth a conversation.

Why Physical Therapists Choose AdvantageCare

AdvantageCare Rehabilitation has been providing rehabilitation services for more than 23 years, and we're proud to be therapist-founded and therapist-led.

That clinical foundation continues to influence how we approach rehabilitation today.

We believe Physical Therapists should have the ability to use their expertise, communicate openly with leadership, collaborate with their team, and focus on helping patients achieve meaningful functional goals.

What You Can Expect

Clinical autonomy. You're the Physical Therapist. Your clinical judgment and expertise matter when determining the best course of treatment for your patients.

A rehab team you'll know. This is a consistent full-time position where you can become an established member of the Pocopson team.

Supportive leadership. Work with a Director of Rehabilitation who is approachable, collaborative, and invested in the success of both the clinicians and the patients they serve.

Interdisciplinary collaboration. Work alongside PTAs, OTs, COTAs, SLPs, nursing and other members of the care team.

The opportunity to see your impact. Follow your patients' progress and see the difference your treatment makes in their mobility, confidence, safety and independence.

Room to grow. Whether your goals are clinical development, mentoring, or eventually moving into leadership, AdvantageCare believes in developing and promoting talent from within.

What Will You Do as a Physical Therapist?

As a Physical Therapist at Pocopson Home, you'll evaluate and treat patients with a variety of mobility and functional needs while developing individualized treatment plans based on each person's abilities and goals.

Your role will include:

  • Performing comprehensive Physical Therapy evaluations
  • Developing individualized plans of care and functional goals
  • Addressing strength, balance, gait, transfers, endurance and functional mobility
  • Providing therapeutic exercise and functional mobility training
  • Developing fall-prevention and safety strategies
  • Assessing the need for walkers and other assistive devices
  • Monitoring patient progress and modifying plans of care when appropriate
  • Supervising and collaborating with Physical Therapist Assistants
  • Educating patients, families and caregivers
  • Collaborating with the interdisciplinary care team
  • Completing accurate and timely clinical documentation

Most importantly, you'll help people work toward something that can completely change their quality of life: the ability to move more safely and independently.

Why Pocopson Home?

For a Physical Therapist who enjoys geriatrics, functional rehabilitation and medically complex patients, Pocopson provides the opportunity to use a broad range of clinical skills.

You'll have the opportunity to work with individuals who may be recovering following hospitalization, illness, surgery, injury, falls, weakness or functional decline.

That means your work can include everything from strengthening and balance to gait training, transfer training, fall prevention and helping someone regain the confidence needed to participate more independently in everyday life.

And because you'll be there full time, you won't simply be covering treatments—you'll have the opportunity to build relationships with your patients and become part of their rehabilitation journey.
