Job Description:

Duties/Responsibilities

  • Maintains accountability for safe, accurate, and effective medication-use processes across inpatient, outpatient, and procedural care settings.

  • Performs prospective and concurrent medication order verification for appropriateness, dosing, route, frequency, interactions, contraindications, monitoring needs, and patient-specific factors.

  • Evaluates medication therapy and implements clinical interventions to optimize medication safety, therapeutic effectiveness, and patient outcomes.

  • Maintains accurate patient medication profiles and supports continuity of care through medication reconciliation, discharge medication review, and patient/caregiver counseling as applicable.

  • Applies age-specific and pediatric medication-use considerations, including weight-based dosing and developmental factors, in patient care decisions.

  • Verifies and authorizes medication dispensing for inpatient, outpatient, procedural, and ambulatory workflows.

  • Directs and prioritizes pharmacy technician workflow to support safe, timely, and accurate medication preparation, distribution, inventory management, and service continuity.

  • Maintains operational oversight of medication-use systems, including Epic Willow, Omnicell, medication builds, dispensing logic, inventory alignment, discrepancies, variances, and system-related alerts.

  • Verifies and, when needed, performs sterile and nonsterile compounding in accordance with organizational policy, USP standards, and applicable regulatory requirements.

  • Ensures compounded products meet requirements for accuracy, stability, compatibility, labeling, documentation, and beyond-use dating.

  • Maintains control and oversight of controlled substances, hazardous drugs, and medication storage areas in accordance with regulatory and organizational requirements.

  • Identifies, evaluates, communicates, and mitigates medication safety risks, including medication errors, near misses, discrepancies, and workflow variances.

  • Participates in medication-use evaluation, quality improvement, regulatory readiness, formulary support, medication stewardship, and practice advancement activities.

  • Communicates effectively with prescribers, nurses, patients/caregivers, pharmacy staff, and other healthcare personnel to support safe and coordinated patient care.

  • Maintains readiness for variable workload, staffing conditions, urgent clinical situations, and workflow disruptions in a non-24/7 pharmacy environment

Required Skills/Abilities

  • Graduate of an ACPE-accredited school of pharmacy.

  • Current pharmacist license in good standing with the Texas State Board of Pharmacy.

  • Maintains active pharmacist licensure, required continuing education, and competency requirements in accordance with Texas State Board of Pharmacy requirements and organizational policy.

  • Completion of an ACPE-accredited sterile compounding (IV) certification program required.

  • Demonstrates and maintains sterile compounding competency in accordance with organizational and regulatory requirements.

  • Hospital pharmacy experience preferred.

  • Demonstrated proficiency in medication order verification, clinical evaluation, dispensing, compounding, medication-use systems, and workflow prioritization.

  • Effective oral and written communication skills, interpersonal skills, computer skills, and ability to manage competing priorities without loss of accuracy or safety.
