The OT role supports that mission by delivering safe, reliable skilled care in the home within OT scope — with excellent communication and escalation to the clinical team so patients stay safe and on track.

What you will do
Complete in-home OT evaluations and deliver skilled OT visits.
Build and execute a clear plan of care focused on ADLs/IADLs, safety/fall prevention, and functional independence.
Recommend and train on adaptive equipment and home modifications as needed.
Coordinate with nursing and the broader care team when condition or needs change.

Requirements
Must-haves
Active Texas OT license (in good standing)
Strong clinical judgment and comfort working independently in the field
Comfort escalating changes in condition to RN/MD and documenting rationale clearly
Strong communication and documentation
Reliable follow-through on orders, frequencies, and patient goals
Current driver’s license and comfort with daily travel across assigned territory
Able to carry/transport basic therapy equipment as needed
Competency in core home health OT scope (ADL/IADL training, home safety, adaptive equipment, caregiver education)
Nice-to-haves
Prior home health experience
Deeper experience in high-signal OT populations (neuro, CVA, fall risk, dementia/caregiver training)
Strong organization and time management
Experience using charting systems / EMRs
