The Mobile Crisis Outreach Team (MCOT) is looking for a Mental Health Case Manager to provide mobile mental health crisis and short-term case management services during business hours Monday through Friday.  MCOT is dedicated to resolving acute and chronic emergency situations by providing brief and intensive services with the goal of reducing the need for hospitalization, diverting from justice system contact, connecting to community services both mental health and otherwise, and meeting basic needs. Johnson County Mental Health provides hope and promotes recovery through the Strengths Perspective. Job Description

Additional duties include:

• Engaging individuals experiencing a behavioral health crisis, building rapport, assessing motivation, and encouraging participation in services.• Carrying a crisis case management caseload; connecting individuals who have multi-faceted needs to ongoing supports and services.• Assessing individuals’ circumstances to identify their needs and determine eligibility for services across a range of areas, including mental health, medical conditions, aging-related needs, intellectual and developmental disabilities, traumatic brain injuries, and basic needs such as food and housing.• Responding quickly in crisis moments to various community locations to provide assessment, intervention, referral, and/or connection to higher level of care as needed.• Working with other Johnson County Mental Health Center (JCMHC) Emergency Services teams and law enforcement to coordinate care for individuals in crisis.• Coordinating with other community agencies to connect clients to needed resources and services.• Completing and submitting timely documentation regarding services provided. • Respecting and maintaining confidentiality as defined under HIPAA and agency guidelines.

Job Requirements

Minimum Job Requirements:

Bachelor’s degree in Human Services or related field is required.  Requires KS/MO driver’s license with an acceptable driving record. 

 

Johnson County Government requires reference/background screening for all positions.  Specified criteria may vary by Department/Agency.

 

Preferred Job Requirements:

One year of related work experience is preferred.

 

Education/Experience Substitutions:

Experience may be substituted for degree. Education may be substituted for experience.

In some instances, statutory or regulatory requirements may prohibit substitution of education and/or experience. 

 

Physical, Environmental, and Special Working Conditions:

All County employees may be called upon to assist other departments in a declared emergency situation.
