As a Licensed Practical Nurse (LPN), you’ll be a valued member of the Acute Care team, working alongside Registered Nurses and other healthcare professionals to provide compassionate, evidence-based care to patients and their families. You’ll help create a safe, supportive and therapeutic environment while playing an important role in each patient’s care journey.

Responsibilities
- Provide hands-on nursing care for patients with physical disabilities and chronic illnesses.
- Assess patient needs and contribute to individualized plans of care, education, and discharge planning.
- Educate patients and families and reinforce care and instructions provided by other team members.
- Provide direction and supervision to unlicensed assistive personnel while delegating appropriately within the LPN scope of practice.
- Collaborate with nurses, providers, and interdisciplinary team members to coordinate patient care and achieve overall goals.
- Communicate effectively with patients, families, and healthcare team members while maintaining accurate documentation.
- Implement the interdisciplinary plan of care to promote recovery, maintain function, and prevent complications or further loss of function.
Qualifications
Education:
- Graduate of accredited LPN program required.
Experience:
- One (1) year of previous CNA/PCT experience preferred.
License(s)/Certification(s):
Current licensure in good standing to practice as a Licensed Practical Nurse in primary state. Must apply within 90 days for licensure in secondary state.
Must possess and maintain current Basic Life Support (BLS) certification or obtain
certification within three (3) months from date of hire.
