Additional Information

Clinic Details

The Mentor Clinic is a demonstration clinic for the “health home” model of primary care. It is intended to be like the school system – if you live in this neighborhood, you come to this clinic for care. You are attached to the clinic, not to an individual healthcare provider, so that if providers go on vacation, retire, leave or are unavailable, you will still get your healthcare services at this clinic.

It is built on a truly collaborative approach to primary care that puts you at the center of a team of dedicated health care practitioners. Our focus is on helping patients to learn and understand how their bodies work so that they can develop their long-term health goals and be an active partner in the care and management of their own health.
