Our home health census is growing, and we’re looking for compassionate and talented Speech-Language Pathologists (SLPs) to join our expanding therapy team in St. Clair Shores, Eastpointe, Grosse Pointe, Monroe, and Belleville!

❤️ What You'll Do

As a member of our home health therapy team, you’ll provide personalized, one-on-one care to help patients improve communication, cognition, swallowing, safety, and overall quality of life in the comfort of their homes.

SLPs will:

  • Complete comprehensive speech-language evaluations to assess communication, cognition, voice, and swallowing needs.
  • Develop individualized plans of care and measurable treatment goals.
  • Provide evidence-based interventions for speech, language, cognitive-communication, and swallowing disorders.
  • Assess and treat conditions including dysphagia, aphasia, dysarthria, cognitive impairment, and other communication disorders.
  • Reassess patient progress and modify treatment plans as clinically appropriate.
  • Provide skilled speech therapy services in accordance with physician orders and CMS Conditions of Participation (CoPs).
  • Educate patients and caregivers on communication strategies, swallowing precautions, aspiration prevention, cognitive techniques, adaptive tools, and individualized Home Exercise Programs (HEPs).
  • Maintain timely, accurate documentation in accordance with CMS, payer, and agency requirements.
  • Collaborate with physicians, caregivers, nursing, therapy, and the interdisciplinary team to support successful patient outcomes.

💼 Our Ideal HHC Guru

You may be our next Home Health Magic Maker if you have:

  • Degree from an accredited Speech-Language Pathology program.
  • Current Michigan Speech-Language Pathologist license in good standing.
  • Certificate of Clinical Competence (CCC-SLP) preferred, as applicable.
  • Home health experience preferred.
  • Strong clinical judgment, critical-thinking, and problem-solving skills.
  • Excellent communication, interpersonal, and patient education skills.
  • Ability to work independently while remaining connected to the interdisciplinary care team.
  • Reliable transportation, valid driver's license, and current auto insurance.
  • Familiarity with Medicare/CMS documentation preferred.
