About the job

The Enrollment MA is a compassionate, self-motivated clinical professional who serves as the primary point of contact for identifying and enrolling patients into Vynca's palliative care program. In this role, you will conduct targeted outreach calls to patients, families, and healthcare organizations — educating them on palliative care and VyncaCare's services, completing structured eligibility screenings to identify patients with chronic illness and functional decline, and scheduling those who qualify for an initial visit with a nurse practitioner. Success in this position requires a strong foundation in motivational interviewing and health literacy-aware communication, as you will often be the first voice guiding patients and caregivers through an unfamiliar but life-changing program. You will build and maintain collaborative relationships with health plans, physician offices, discharge planners, and community partners to drive census growth and expand program reach. Alongside your direct outreach, you will be expected to contribute qualitative and quantitative feedback on outreach trends and barriers, working closely with the market and business development team to inform strategy.

What you’ll do

  • Conduct targeted telephonic outreach to patients on provided lists, following Vynca's standard messaging while delivering the highest standard of customer service and patient-centered communication

  • Confirm service area eligibility and complete structured screening forms capturing demographic information, chronic disease history, and indicators of functional decline to determine program eligibility

  • Educate patients and families on palliative care, Vynca 's services, and what to expect from the program — translating complex clinical concepts into accessible, reassuring language

  • Schedule eligible patients for an initial visit with a nurse practitioner and coordinate logistics to ensure a smooth handoff from enrollment to care

  • Act as a liaison between patients and the broader care team, directing calls, texts, and emails to the appropriate individuals or departments and responding to patient questions and concerns in a timely, supportive manner.

  • Maintain confidentiality and accuracy in all patient documentation and communications in accordance with HIPAA and organizational standards

  • Coordinate enrollment efforts across targeted regions, tracking outreach activity systematically to encourage referrals and support census growth

  • Identify outreach trends, surface barriers to enrollment, and share qualitative and quantitative feedback and creative ideas with the market and business development team to continuously improve program reach

  • Maintain a strong, collaborative working relationship with your designated market team and practice leadership, contributing to shared goals around program expansion and patient access

Your experience and qualifications

  • Current CMA or CCMA certificate issued by a recognized credentialing body

  • 2 years of healthcare experience required; 8 months of previous clinical experience preferred

  • 1 year of telephonic or call center customer service experience; call center experience highly preferred

  • Experience working in a medical practice or healthcare setting, including familiarity with EHR platforms for documenting patient interactions and scheduling appointments

  • Experience and training in motivational interviewing and patient education. Comfort discussing serious illness, chronic disease, and end-of-life adjacent topics with patients and families

  • Strong organizational skills with the ability to work independently and manage a high-volume outreach workload

  • Proficient with Microsoft and Google applications; must have access to a reliable internet connection

  • Strong written and verbal communication skills

  • Exceptional interpersonal skills including conflict resolution and de-escalation techniques

  • Willing and able to work Monday through Friday, 8:30am–5pm Eastern Standard Time, with the possibility of overtime

Preferred Qualifications

  • Bilingual (English/Spanish)

  • Medical terminology certification

  • Familiarity with palliative care, home-based care models, or chronic disease management
