Principales responsabilités
- Accompagner, encadrer et soutenir les usagers au sein du centre de réadaptation afin de favoriser leur autonomie et leur intégration sociale
- Évaluer les besoins des personnes visées, concevoir et appliquer des plans d'intervention éducatifs adaptés
- Collaborer activement avec l'équipe interdisciplinaire et les proches des usagers pour assurer la cohérence des interventions
- Assurer une documentation rigoureuse des observations et de l'évolution des dossiers cliniques conformément aux normes du milieu
