A Brief Overview
This role provides diabetes self-management education and support for patients with Type 1 and Type 2 diabetes. This role also develops and maintains related curriculums and systems.

What you will do

  • Provide patient education and ongoing patient support in consultation with physician and advanced practice nursing staff

  • Perform complete educational and skills assessments for patients with Type 1 and Type 2 diabetes managed on oral medications, insulin injections and insulin pumps

  • Provide diabetes self-management education to families including 1:1, group / classroom settings and electronically (telephone/web-based)

  • Develop, document and implement education & treatment plan for patients

  • Evaluate and meet patient and family learning needs

  • Follow up with patients to evaluate achievement of learning goals

  • Assist families with making decisions about diabetes self-care including meal planning / carbohydrate counting, planning for exercise, daily schedule adjustments, responding to high and low blood sugar readings and other questions

  • Determine insulin dosing based on blood glucose patterns for patients on insulin pumps, on basal/bolus programs and on multiple daily injections (MDI)

  • Assist with family sick day management guidelines and identify / coordinate resources

  • Develop and maintain patient education curriculums and systems necessary to maintain Americans with Disabilities Act (ADA) recognition

  • Develop and revise educational materials for patients and clinics

  • Collaborate internally regarding patient education systems (Connelly, Homecare)

  • Participate in data collection and analysis for ongoing accreditation processes

  • Participate in reviews of program, system and patient outcomes to make program changes using data including volume, revenue, safety, outcomes, and satisfaction.

  • Develop resources for clinicians including technology updates and practice guidelines

  • Collaborate with clinicians on continuous glucose monitoring systems including operation, sensor device insertion and downloading / interpretation of monitoring data

  • Assist in product incorporation and act as industry liaison for diabetes supplies including meters, pumps, continuous glucose monitoring and insulin products

Education Qualifications

  • Bachelor's Degree Required

Experience Qualifications

  • At least one (1) year pediatric diabetes-related experience Preferred

  • managing patients on insulin pumps and use of CGMS systems Preferred

Licenses and Certifications

  • Registered Nurse (Pennsylvania) - Pennsylvania State Licensing Board - upon hire - Required or

  • Licensed Dietitian-Nutrition (LDN) (Pennsylvania) - Pennsylvania State Licensing Board - upon hire - Required

  • Certified Diabetes Educator (CDE) - Certification Board for Diabetes Care and Education (CBDCE) - within 12 months - Required

SALARY RANGE:

$83,700.00 - $106,700.00 Annually

Salary ranges are shown for full-time jobs. If you're working part-time, your pay will be adjusted accordingly.
