Reporting to the Home Care Coordinator and Nurse, the Continuing Care Assistant (CCA) is an integral member of the Home Care Team, providing personal care, respite, palliative care, and home management support services to clients in their homes and within the community.

The Continuing Care Assistant works collaboratively with clients, families, healthcare providers, and community partners to support clients in maintaining their independence, dignity, safety, and quality of life. The CCA provides compassionate, respectful, and person-centred care while recognizing the importance of Mi'kmaq culture, traditions, family, and community in supporting holistic wellness.

Key Responsibilities

  • Provide personal care and home management support services to Eskasoni Home Care clients in accordance with established care plans.
  • Promote client independence, dignity, choice, and quality of life while providing care and assistance.
  • Provide respite care and palliative care as required.
  • Assist clients with activities of daily living and other care needs within the scope of the CCA role.
  • Develop and maintain positive, respectful, and professional relationships with clients and their families.
  • Work collaboratively with the Home Care Coordinator, Nurse, healthcare providers, colleagues, and community partners to support coordinated client care.
  • Observe and report changes in a client's health, functional status, behaviour, or overall well-being to the appropriate supervisor or healthcare provider.
  • Keep the supervisor informed of health and safety concerns, client needs, requests, and other issues that may affect the client's care.
  • Follow established client care plans, direction from the supervising nurse or coordinator, and applicable policies and procedures.
  • Maintain accurate and confidential client information and documentation in accordance with organizational policies and privacy requirements.
  • Maintain strict confidentiality regarding client, family, employee, and organizational information.
  • Identify and report safety concerns within the client's home or care environment.
  • Support clients in maintaining a safe, clean, and comfortable home environment within the scope of the position.
  • Demonstrate respectful and culturally appropriate communication with clients, families, colleagues, and community members.
  • Participate in team meetings, training, continuing education, and professional development activities as required.
  • Adhere to all policies, procedures, standards, and guidelines of Eskasoni Health Centre and Eskasoni Band Council.
  • Carry out additional duties as required by the Home Care Coordinator, Nurse, or leadership.

Qualifications

  • Successful completion of a recognized Continuing Care Assistant (CCA) Certificate program.
  • Current CPR and First Aid certification.
  • Current WHMIS certification.
  • Current Food Handler certification.
  • Valid Nova Scotia driver's license and access to a reliable vehicle.
  • Strong communication, interpersonal, organizational, and time-management skills.
  • Ability to work independently while also functioning effectively as part of a multidisciplinary and community-based team.
  • Compassionate, respectful, dependable, and positive approach to client care.
  • Ability to maintain professional boundaries and client confidentiality.
  • Ability to follow care plans, policies, procedures, and direction from supervisors and healthcare professionals.
  • Ability to speak and/or understand Mi'kmaq is considered an asset.

Experience

  • Experience working as a Continuing Care Assistant or in a home care, long-term care, community care, or healthcare setting is preferred.
  • Experience providing personal care and assistance with activities of daily living is preferred.
  • Experience providing respite or palliative care is considered an asset.
  • Experience working with Indigenous communities is considered an asset.
  • Experience working with older adults, individuals with disabilities, chronic health conditions, or individuals requiring supportive care is considered an asset.
  • Experience working collaboratively with families, healthcare providers, and community services is considered an asset.

Cultural Knowledge & Skills

  • Knowledge and understanding of Mi'kmaq culture, traditions, values, and community practices.
  • Demonstrated respect for Indigenous ways of knowing, cultural practices, family structures, and community relationships.
  • Ability to provide culturally safe, respectful, and person-centred care.
  • Ability to build trusting and respectful relationships with clients, families, and community members.
  • Understanding of the importance of supporting client independence, dignity, family involvement, and holistic wellness.
  • Commitment to providing care that is responsive to the needs, values, and preferences of Eskasoni community members.

Additional Requirements

  • Current and original Vulnerable Sector Check, Criminal Record Check, and Child Abuse Registry Check, or ability to obtain the required checks prior to employment.
  • Current CPR, First Aid, WHMIS, and Food Handler certifications.
  • Valid driver's license and access to a reliable vehicle.
  • Ability to travel within the community as required to provide home care services.
  • Willingness to participate in required training, continuing education, and professional development.
  • Ability to work flexible hours as required by client and program needs.
  • Commitment to maintaining confidentiality and complying with all applicable privacy, health and safety, and organizational policies.
