Position Purpose

This role strengthens Carlisle’s community support system by:

  • Improving access to human services and basic needs resources
  • Supporting residents through coordinated referral and follow-up
  • Enhancing inclusive access to recreation programs
  • Strengthening collaboration across Town departments and schools
  • Supporting early identification of resident needs through multiple points of contact
  • Building a more connected and responsive community services system


Position Summary

The Community Services Coordinator is a Grade 7 position responsible for coordinating human services support and inclusive recreation access across the Council on Aging and Human Services (COAHS) and the Recreation Department.

The position emphasizes timely response to resident needs through cross-department coordination and the use of established referral systems and professional judgment to connect individuals with appropriate services. It provides direct assistance to residents navigating community resources while also supporting inclusive participation in Town recreation programs for individuals with a range of support needs.

The position functions as a key cross-departmental connector, ensuring coordinated, consistent, and accessible services for residents of all ages. Work is performed with a high degree of independence, following established policies, professional standards, and departmental guidance, while requiring sound judgment in applying resources and determining appropriate referrals and supports.


Essential Functions


Human Services & Community Support (Approx. 20 Hours)


Resident Services & Resource Navigation

  • Provides intake, information, referral, and coordination services for residents of all ages.
  • Assists with fuel assistance, rental assistance, food insecurity, and other basic needs supports.
  • Connects residents to appropriate local, regional, and state resources based on identified needs.
  • Supports seniors, caregivers, and families in accessing services that promote stability, independence, and aging-in-place.
  • Maintains confidential documentation and case notes in accordance with Town procedures.


Outreach, Follow-Up & Interdepartmental Coordination

  • Conducts proactive outreach and ongoing follow-up with residents to support continuity of care, stability, and connection to community resources.
  • Collaborates with Police, Fire, Board of Health, schools, and other Town departments to identify residents who may be experiencing isolation, unmet needs, or emerging challenges before situations escalate into crisis.
  • Coordinates referrals, case discussions, and interdepartmental responses to address concerns related to health, safety, housing instability, caregiver strain, and social isolation, helping ensure residents receive timely and appropriate support.


Support Groups & Community Services

  • Coordinates caregiver support and bereavement group programming.
  • Serves as a consistent point of contact for human services inquiries, resource navigation, and follow-up support.
  • Supports continuity of care by maintaining awareness of ongoing resident needs and coordinating appropriate internal and external supports.
  • Participates in COAHS meetings and contributes to service planning and coordination activities.

 

Friendly Visitor Program

  • Provides administrative and coordination support for the Friendly Visitor Program as a supplemental outreach and engagement resource.
  • Supports volunteer matching, communication, and coordination in collaboration with COAHS staff.
  • Uses program participation as one of multiple touchpoints to identify resident needs, monitor well-being concerns, and support appropriate follow-up or referrals.


Inclusive Recreation & Community Access (Approx. 15 Hours)


Inclusive Programming Support

  • Supports the Recreation Department in delivering inclusive and adaptive programming for children, teens, adults, and seniors.
  • Provides on-site program support to assist participants who require additional accommodations or structured support to fully participate.
  • Assists recreation staff in implementing inclusive practices and de-escalation/support strategies as needed.
  • Promotes access to Town recreation programs for individuals with diverse developmental, emotional, behavioral, or physical needs.


School & Community Collaboration

  • Collaborates with Carlisle Public Schools, CPAC/SEPAC, families, and community partners to strengthen inclusive participation pathways.
  • Supports coordination between school-based supports and after-school recreation programming.
  • Helps identify barriers to participation and supports practical solutions to increase access and engagement.
  • Participates in regional collaboration opportunities related to inclusive recreation as appropriate.


Program Support

  • Assists with the planning, coordination and supervision of year-round recreation programs.
  • Provides direct support during after-school and evening recreation programming, generally between 3:00 PM and 6:00 PM.
  • Assists with seasonal programming, including summer recreation programs and special events.
  • Assists with supervision of program instructors and seasonal part-time staff.
  • Supports the Recreation Director in maintaining safe, inclusive, and structured program environments.


Qualifications

  • Bachelor’s degree in human services, social work, recreation, psychology, education, or related field preferred
  • Experience in human services, case coordination, recreation programming, or disability support strongly preferred
  • Strong communication, organizational, and interpersonal skills
  • Ability to work effectively across departments, schools, families, and community partners
  • Experience working with individuals with diverse support needs preferred
  • CPR/First Aid certification preferred
  • Valid driver’s license required
