Join our Lakes team as a part-time Community Nurse and make a meaningful difference in the lives of individuals, whānau, and communities across the region.


In this role, you'll provide comprehensive, person and whānau-centred nursing care across a range of settings, including clinics, homes, schools, community locations, and telehealth. Working collaboratively with general practice teams, community providers, iwi and Māori health providers, specialist services, and wider health and social support agencies, you'll help improve health outcomes, access to care, self-management, and equity.


You'll undertake health assessments, care planning, clinical management, health promotion, education, care coordination, and navigation across community and primary care settings. You'll also support integrated care delivery through outreach services, nurse-led clinics, and multidisciplinary collaboration, while acting as an expert nursing resource and advocate for patients and whānau.


The ideal candidate is a confident and experienced nurse who enjoys working autonomously, building strong relationships, and delivering high-quality care that makes a lasting impact in the community.

He kōrero mōu: About you

  • Registered Nurse with a current New Zealand Annual Practising Certificate (APC).
    • Minimum of five years post-registration nursing experience.
    • Demonstrated experience in primary care, community nursing, or chronic condition management.
    • Excellent clinical assessment, care planning, and nursing management skills.
    • Experience working autonomously and collaboratively within multidisciplinary teams.
    • Strong care coordination, clinical reasoning, and problem-solving abilities.
    • Experience providing education to patients, whānau, and healthcare professionals.
    • Understanding of population health, health equity, and Te Tiriti o Waitangi principles.
    • Excellent communication, relationship-building, and organisational skills.
    • Competency using clinical management systems and Microsoft 365 applications.
    • Current full New Zealand driver's licence.


Desirable:

  • Qualified Registered Nurse Prescriber (Designated Prescriber) or Community Nurse Prescriber.
    • Postgraduate nursing qualification or relevant specialty qualification.
    • Experience managing long-term conditions such as diabetes, cardiovascular disease, and respiratory conditions.
    • Experience delivering outreach and community-based services.
