🧡 What You'll Do

• Assist patients with activities of daily living (ADLs), including bathing, grooming, dressing, oral care, toileting, and feeding
• Provide comfort measures and emotional support to patients experiencing advanced illness and end-of-life care
• Reposition and transfer patients safely to promote comfort and prevent skin breakdown
• Assist with ambulation and mobility according to patient needs and care plans
• Observe and promptly report changes in patient condition, behavior, or comfort level to nursing staff
• Maintain patient dignity, privacy, and respect at all times
• Ensure patient rooms are clean, organized, and stocked with necessary supplies
• Change bed linens and help maintain a comfortable and safe environment
• Clean and disinfect equipment according to established procedures
• Assist with admissions, transfers, and discharge preparation
• Welcome and support family members and visitors with compassion and professionalism
• Provide comfort items and supportive services to patients and families
• Work collaboratively with nurses, social workers, chaplains, volunteers, and other interdisciplinary team members
• Follow individualized patient care plans and hospice policies

• CNA certification or HHA credential preferred
• Previous caregiver or healthcare-related experience preferred
• High School Diploma or equivalent
• Ability to read, write, and communicate effectively in English
• Ability to understand and follow written and verbal instructions
• Compassionate, patient-centered approach to care
• Strong communication and interpersonal skills
• Strong organizational and time management skills
• Ability to handle sensitive end-of-life situations with empathy and professionalism
• Strong observation and reporting skills
• Team-oriented mindset
• Physical ability to assist with lifting, turning, transferring, and repositioning patients
• Ability to maintain a safe and clean environment
• Proof of Influenza Vaccination or approved Medical or Religious Exemption
• Employees required to travel between sites must maintain a valid driver's license and auto insurance
