Patient Care Coordination

• Coordinate patient care activities from admission to discharge.
• Facilitate communication among consultants, nursing teams, diagnostic services, and support departments.
• Ensure timely scheduling of investigations, procedures, and consultations.
• Monitor patient progress and escalate concerns to the treating team when required.

Clinical Operations

• Support multidisciplinary rounds and coordinate follow-up actions.
• Ensure implementation of treatment plans prescribed by clinicians.
• Track critical patients and ensure continuity of care.
• Coordinate referrals and transfers within or outside the hospital.

Patient Experience & Satisfaction

• Address patient and attendant concerns promptly and effectively.
• Provide patients and families with information regarding treatment plans, procedures, and hospital services.
• Ensure a positive patient experience throughout the care journey.
• Conduct patient feedback collection and assist in service recovery initiatives.

Documentation & Compliance

• Maintain accurate documentation of patient care coordination activities.
• Ensure compliance with NABH, hospital policies, and quality standards.
• Assist in clinical audits and quality improvement initiatives.
• Coordinate insurance and discharge-related documentation, as applicable.

Interdepartmental Coordination

• Liaise with laboratory, radiology, pharmacy, billing, and other clinical departments to facilitate patient care.
• Coordinate discharge planning and ensure timely discharge processes.
• Support bed management and patient flow optimization.
