What You'll Do

• Partner with insurance companies and payers to advocate for appropriate rehabilitation and medical services• Conduct comprehensive assessments of patients' medical, social, and psychosocial needs• Collaborate with interdisciplinary team members to develop individualized care plans• Coordinate and monitor rehabilitation, medical, and support services• Advocate for patients and ensure their needs are addressed throughout the recovery process• Provide education, resources, and support to patients and their families• Facilitate communication between patients, families, providers, and community resources

Responsibilities

 

What you will do: Responsibilities listed include but not limited to:

• Work with insurance and payers to advocate for the appropriate rehabilitation and medical care. 
• Conduct comprehensive assessments of patients' medical, social, and psychological needs
• Collaborate with the treatment team to develop individualized care plans.
• Coordinate and monitor the delivery of services, including therapy, medical care, and support services
• Advocate for patients' rights and ensure their needs are met throughout their rehabilitation journey
• Provide education and support to patients and their families

Qualifications

What you will need:

• Bachelor's or Master's degree in a related field preferred (e.g., Social Work, Psychology)
• Clinical case management experience, preferably in a rehabilitation or healthcare setting
• Strong interpersonal and communication skills
• Knowledge of brain injuries and rehabilitation preferred

Salary Range

USD $61,700.00 - $72,800.00 / Year
