Overview:

Sentara Health is seeking to hire a qualified individual to join our team as a Care Transition Coordinator.

Position Status: Full Time - Day Shift

Position Location: Hampton, VA at Careplex Hospital

Overview

Responsible for the identification and assessment of home care and hospice needs of patient population and coordination of these services to ensure a smooth transition to the next site of care.

Certification/Licensure

• Licensed RN, LPN, RT or BSW required. Proof of education as LPN, RN, RT or BSW required.

Experience

• 1 year of home care, discharge planning or case management experience required.

We provide market-competitive compensation packages, inclusive of base pay, incentives, and benefits. The base pay range for full-time employment is $76,793.60 - $117,603.20 annually. Additional compensation may be available for this role, such as shift differentials, on-call pay, shift premiums, extra-shift incentives, or bonus opportunities. Compensation within the range may vary based on qualifications, experience, location, market conditions, and business needs. Note: If an annual salary is posted, it is based on a full-time colleague working 2,080 hours annually. Sentara CarePlex Hospital, located in Hampton, VA, is a 224-bed technologically advanced, acute care facility, and Certified Primary Stroke Center.
