This highly collaborative position combines patient engagement, care coordination, and population health management to help patients navigate complex healthcare needs, improve access to care, and achieve better health outcomes.

  • Coordinate referrals and transitions for patients initiating outpatient dialysis services.

  • Gather and organize clinical and non-clinical information from patients, caregivers, providers, electronic health records, and other systems to support care management activities.

  • Collaborate closely with multidisciplinary care teams to identify opportunities for intervention and improve patient outcomes.

  • Serve as a resource for patients, physicians, caregivers, and internal stakeholders regarding CKCC program services and processes.
