About this role

Epic Health Partners, LLC is seeking a dedicated ACT Clinician to join our growing Assertive Community Treatment (ACT) team in Danville, VA. The ACT Clinician provides community-based mental health services to adults with serious and persistent mental illness. Working as part of a multidisciplinary team, this position provides assessment, treatment, rehabilitation, care coordination, and crisis support services to help individuals achieve stability and recovery. This role is ideal for a clinician who is passionate about helping individuals succeed in their communities and enjoys working in a team-based environment.


Essential Duties & Responsibilities: 

  • Conduct assessments and participate in the development and implementation of individualized treatment plans.
  • Provide counseling, rehabilitation, crisis intervention, and support services to individuals with serious mental illness.
  • Deliver services in community settings, including individuals' homes and other natural environments.
  • Coordinate care with ACT team members, medical providers, family members, and community resources.
  • Assist individuals in developing skills that support recovery, independence, and community integration.
  • Maintain timely and accurate clinical documentation in accordance with regulatory and agency requirements.
  • Assist with weekly medication bag and box fills.
  • Deliver medications during scheduled morning and evening medication runs on a rotating basis.
  • Attend team meetings, trainings, and treatment planning sessions.
  • Complete required documentation in a timely manner, including progress notes, treatment plans, quarterly reviews, and service transitions.

Qualifications:

  • Master’s Degree in Counseling, Social Work, Psychology, or a related field.
  • Licensed or license-eligible in Virginia as LPC, LCSW, LMFT, or Clinical Psychologist.
  • Minimum of 1 year of clinical experience including assessments, treatment planning, therapy services, and crisis/behavioral interventions.
  • Valid driver's license and safe driving record required.
    Must obtain or be eligible to obtain Virginia Pre-Screener Certification for Emergency Hospitalization within six months of hire.
  • Strong knowledge of VADBHDS and DMAS regulations and evidence-based practices.
  • Proficiency in psychosocial assessment, treatment planning, and clinical documentation.
  • Strong verbal and written communication skills.
  • Effective critical thinking, problem-solving, and decision-making abilities, including in crisis situations.
