BAYADA is seeking compassionate, dedicated, and experienced Admissions Registered Nurses (RNs) to join our Ocean County Home Health team. If you're passionate about providing exceptional one-on-one patient care and enjoy the autonomy and flexibility of home health, we'd love to meet you.
Experience with OASIS documentation and conducting Start of Care visits is required.
Full-time RN Case Management opportunities are also available throughout Ocean County.

Admissions RN Responsibilities:
- Complete comprehensive Start of Care (SOC) and admission visits for home health patients.
- Perform thorough patient assessments and accurately complete OASIS documentation.
- Develop individualized plans of care in collaboration with physicians and the interdisciplinary team.
- Deliver skilled nursing care, including medication administration, wound care, treatments, and patient education.
- Monitor patient progress and communicate changes in condition to the care team.
- Educate patients and caregivers to promote independence, safety, and successful outcomes at home.
- Perform approximately 1–3 Start of Care visits per day within a designated territory, allowing for quality one-on-one care.
Qualifications:
- Current New Jersey Registered Nurse (RN) license in good standing.
- Minimum of one year of nursing experience; home health experience preferred.
- Experience with OASIS documentation and completing Start of Care visits required.
- Comfortable utilizing an EMR; HCHB experience is a plus.
- Excellent assessment, communication, critical thinking, and organizational skills.
- Reliable transportation and willingness to travel throughout Ocean County.
Additional Information:
Base Pay, depending upon qualifications and subject to negotiation:
$55.00 - $62.00 per point
